



ENGAGEMENT PROJECT
FOR THE COMMUNITY COUNCIL OF STAFFORDSHIRE
During the next six months the Community Council of Staffordshire will be undertaking on behalf of, and in liaison with, Staffordshire Local Involvement Network (LINK), a countywide community engagement. This will be a focused consultation programme with existing networks, groups and individuals in order to ascertain people’s views, concerns and experiences of health and social care services throughout the county and to find out how people believe these might be improved or changed to benefit their communities.
As part of this exercise, we have produced a short questionnaire (click here to download) about accessing health and social care services in Staffordshire.
Whilst the questionnaire concentrates on the following eight questions, we are also interested in any other thoughts or comments you have on health and social care services, for instance where there are gaps in provision.

2. Did you find it easy to get there?
3. Did you have to wait long for an appointment or to be seen on the day?
4. Do you have a disability of any kind? Did this make your experience of
using the service more difficult?
5. Did you feel you were treated with dignity and respect by the people you
came into contact with?
6. How would you rate the overall quality of the service you received?
